What Is Home Health Care

Home health care encompasses a wide range of health and social services. These are services delivered to recovering, disabled, or chronically ill persons in need of some of the following: medical, nursing, social services, therapeutic treatment, or assistance with the essentials of daily living.

Who Needs Home Health Care

Any one who is challenged by a variety of problems such as short or long term illness and/or physical disability. Also included are individuals who elect to live independent lives outside of a hospital or nursing home. These are people who want to take advantage of home health care services as their physical capabilities diminish. Typical home care clients might include the following:

	Individuals discharged from the hospital, recovering from surgery or illness.
	Individuals with a chronic condition that needs frequent monitoring such as diabetes, hypertension, kidney disease, heart/lung disease, or multiple sclerosis.
	Individuals with terminal illness who need a program of pain management, emotional support, personal care or assistance with the activities of daily living.
	Other services include individuals who need custodial care, homemaker services or a companion both temporary or on a long term live in basis.

Home Health Services can contribute to the health and well-being of the patient and can significantly relieve the pressure felt by all family members . Home health services can assist in restoring the patient to health and/or to maximum functioning at home or in familiar surroundings. Home health services can help to prevent costly and inappropriate admissions to the hospital; and allow the patient to get individual attention from nurses, therapists, home health aides and social workers.
